This is a one year contract position funded by the United Way.
Full time position: 30-35 hours per week (with flexible hours per week)
The details of the position may change but essentially includes but not
limited to the following.
Duties:
Working with Executive Director, other PHC team members, and members of the Community
Health and Wellness Collaborative design and develop Gabriola’s seniors’ prescribing model and implementation plan
Identify processes for referral and information exchange between participants and community
Organizations and PHC staff
Schedule Live Life to Full, Living with Chronic Conditions and University of Victoria Telephone
Support System
Establish evaluation criteria, track and prepare reports using output/ measurements
Create new innovative communications for wellness such as podcast or facebook
Establish working relationship with Better at Home, Island Health, Community Paramedics
Facilitate meetings to improve health system referrals
Qualifications:
Degree in Social Work or equivalent community development academic program
Track record in program development using components such as; CBT self care training
Programs with UVIC, CMHA representatives, referrals from physicians, tracking, evaluating, reporting
Ability to support vulnerable seniors
Liaise with community organizations, extending participation opportunities for seniors
Skills:
Compassionate communications techniques
Group Facilitation skills especially with interagency workflows, process improvement, and system changes
Ability to work with complex system change initiatives
Program development
Expert Computer skills
First Aid
Must have a driver’s license and be a resident of Gabriola Island
