What is Social Prescribing?

Social prescribing supports seniors’ health and well-being by connecting them with community programs, activities, and services. These connections help reduce isolation, build relationships, and improve overall quality of life.

This may include:

  • Social groups and community activities

  • Movement and wellness programs

  • Volunteer opportunities

  • Transportation supports

  • Food programs

  • Other local services

How We Can Help?

As a Seniors Connector, we work one-on-one with seniors to help them find the right supports and opportunities.

Together, we can:

  • Explore interests, needs, and goals

  • Navigate available community resources

  • Connect to programs and activities

  • Follow up to ensure the connection is a good fit

This process usually begins with a short, informal conversation to see what might be helpful. In addition to one-on-one support, we also develop and lead social programs for seniors.

Social prescribing may be a good fit for seniors who are:

  • Feeling isolated or looking for more social connection

  • Interested in joining community programs or activities

  • Open to exploring new supports and opportunities

 

PLEASE NOTE: 
Seniors with urgent medical, mental health, or crisis-related needs should be supported through appropriate healthcare or emergency services.​​​​

How To Get Connected?

The best way to get started is for seniors to reach out directly when they feel ready. This helps ensure the timing is right and that they’re open to exploring options.

If you are supporting someone who may benefit:

  • Please share contact information and encourage them to reach out

  • If they prefer a call, you may pass along their name and number

  • It helps if they are aware and expecting the call

We’re also happy to provide guidance if you’re unsure whether social prescribing is the right fit.
