Annual Salary Range: $27-34 hour

The Seniors Community Coordinator is responsible for providing, through partnership with community organizations, non-medical support to improve the overall health and well-being of older adults in our community. This role plays an integral part in bridging the gap between healthcare and social care and includes a diverse range of responsibilities. From conducting assessments to co-developing personalized care plans, to providing practical support to older adults and their families, to establishing, and maintaining relationships with people and organizations in the community offering supports to older adults, this position will make a positive impact on the lives of older adults and the community. This position reports to the Seniors Program Manager.
About Us:
People for a Healthy Community (PHC) exists to provide services and support to residents of Gabriola, Mudge, and DeCourcy Islands throughout their lives, and especially at vulnerable points to improve their health and quality of life in a manner that builds community, maintains dignity and challenges inequities.
Through our own initiatives and in collaboration with others, we deliver programming that includes:

Food programs, ranging from elementary nutrition to family grocery support to seniors’ lunches

Seniors’ programs ranging from adult activity days to aging in place support

Providing access to services such as advocacy, caregiver support, hospice, physical and mental health, and transportation.​

Specific duties include, but are not limited to:

Building and maintaining networks within community and strengthen relationships within the community-based senior serving sector;

Using an asset based community development approach to identify and mobilize individuals and organizations providing for, and supporting, older adults;

Connect, liaise, and establish partnerships with local health care professionals to create and maintain referral pathways;

Maintain an active caseload of seniors with short-term needs through referrals from health care professionals and community agencies;

Prioritize referrals to meet individual participant’s needs;

Complete intake process and use various assessment tools to assess strengths, needs, abilities, and risks;

Interpret participants’ complex physical requirements and social needs;

Develop, and modify when needed, individualized wellness plans using a ‘what matters to you’ approach;

Maintain a database of participant interactions, wellness plans, reports, etc

Maintain a high level of confidentiality in all matters related to program participants and community partners

Engage and participate in learning opportunities such as in community of practice, planning tables, and networking events;

Support all seniors programming in planning and delivery;

Support the Seniors Program Manager in the training, coordination, and supervision of volunteers;

Identify areas of need through observation and feedback and assist in new program development

Education & Experience

Bachelor’s degree in social service or related human services field or a combination of relevant education and experience

Minimum of two years recent related work experience

Strong knowledge of seniors’ issues and challenges related to healthy aging
-
Experience working with seniors and diverse populations from different cultures and socio-economic backgrounds

Knowledge of community resources, programs, and services on Gabriola

Strong collaborative skills and proven ability to establish and maintain effective working relationships with all internal and external contacts

Strong technical skills in G-Suite and the ability to learn new systems

Exceptional communication and interpersonal skills

Analytical thinking and problem solving skills
-
Muti-tasking and time management skills and an ability to work independently and self-start
-
Previous experience working in a not-for-profit environment is an asset
