Seniors Wellness Coach and Respite Program Coordinator
The Seniors Wellness Coach and Respite Program Coordinator is responsible for coordinating wellness program development, support for caregivers and a respite care roster. This position embodies two important functions - encouraging all seniors to become active in managing their aging and assisting caregivers and their partners by providing system navigation, emotional supports, personal wellness and respite care coordination. The Coordinator meets one-on-one with the senior or caregiver to assess the needs and acts as a coach/mentor, developing individual plans of action, following up with plans and removing roadblocks. The Coordinator will also provide the caregiver options for their own specific challenges in caring for their family members.
Key Characteristics and skills:
Experience working with older adults (65+)
Knowledge of physical activity programs and resources in local communities suitable for your specific target population of seniors
Knowledgeable about physical activity and health
Active listening skills
Motivational interviewing skills
High degree of independence and initiative within a team environment
Additional skills and requirements:
Relevant fitness qualifications and experience involving one to one training and support
Knowledge of chronic conditions affecting older adults (heart disease, diabetes, arthritis, stroke, etc.)
Experience facilitating groups using adult education methodologies
Exceptional organizational, interpersonal, communication and conflict resolution skills
Computer proficient – Microsoft Office (Excel, Word, Outlook, PowerPoint)
Demonstrated ability to make sound judgments and decisions; effective prioritizing and problem solving skills
Ability to work some evenings, weekends and flexible schedule, as required
Current and satisfactory Criminal Reference Check/Vulnerable Sector Search issued no later than six (6) months preceding your start date
Current Standard First Aid and CPR Level “C” with AED
Receive referrals from the island family physicians, island health professionals or from PHC other programs or calls from community seniors who self-identify.
Develop Roster for Home Respite Care (Community Health Workers)
Assist with interview, selection and monitoring of respite care staff.
Establish appropriate matching of staff and specific participants, creating a respite plan.
Develop in home activities and ensure safety of both participant and staff and monitor respite client as needs progress.
Ensure VHA expectations for home respite care are being met and consistent with provincial regulations, providing reports as requested.
Program Development and Communications
Participate in BC Community Caregiver Networks so that best practice and new ideas can be implemented.
Support caregivers by providing peer support, personal system navigation and family facilitation.
Develop appropriate handouts explaining the aim of the programs and associated island and provincial services
Manage expectations and work with other care team members so that decisions regarding supports are transparent, rational, and fair.
Work with other community groups to support existing and establish a range of activities geared towards healthy active aging. Follow and develop the Active Aging BC Society program design and provide reports as required.
Supervise staff ensuring the staff are qualified to perform required activities and hold the proper certification.
