Position Available: Caregiver Support Coordinator

Join the PHC seniors program team as the Caregiver Support Coordinator. This position is regular part-time 10 hours per week, with a salary range of $25-30 per hour.

The Program Facilitator, is the coordinator for PHC’s community-based family caregiver support group. The facilitator provides leadership at group meetings to ensure: confidentiality; opportunities to share and to have discussion; avoidance of judgemental attitudes and unsolicited advice and coordinates program development and respite care roster.

Support groups are based on a self-help mutual aid model. The groups provide an opportunity for participants to share thoughts, feelings, information, and conflicts they are experiencing while providing care and support to an adult family member or friend, at home or in a care facility, who may be in poor health, elderly or has a disability.

The program facilitator also meets one-on-one with the caregiver to assess the needs of the family member and determine which community health worker would best fit for respite care; conduct a home assessment to ensure safety for the family member and respite worker. The facilitator will also provide the caregiver options for their own specific challenges in caring for their family member, and assist with system navigation.
